Golden Fish & Chips Takeaway, a takeaway at 104 Carholme Road, Lincoln was given the score after assessment on December 4, the Food Standards Agency's website shows.
It means that of Lincoln's 111 takeaways with ratings, 89 (80%) have ratings of five and just one has a zero rating.
